Here at Ascend Health we do what we can to make the process of starting your recovery simple and straightforward. Starting treatment with us does require some basic information so that we can appropriately serve you as a healthcare provider.
If you have never received treatment for OUD (Opioid Use Disorder) or have not been to a detox facility, you will be subject to the buprenorphine induction process.
Please note that your first visit will take several hours due to this process to ensure your safety when taking buprenorphine for the first time.
Please arrive at your first appointment 30 minutes early to allow time to complete the intake forms. If you’ve filled out the paperwork at home, don’t forget to bring it with you. Forms must be completed before your appointment start time to be seen as scheduled.
Returning Patients
In the event you were formally discharged from our program for non-compliance, you will not be eligible to re-establish. However, if at the time of your discharge you were asked to attend in-patient treatment and have done so with successful completion. The provider may make an exception in these circumstances.

Valid Photo ID (Passport, Driver’s License, Military ID)
Insurance Card (If you don’t have insurance, we do also accept Self-Pay)
